Adult & Children Blended Case Management
The Blended Case Management Program is provided in the community and is designed to assist individuals to access needed community services. Target population is children and adults with mental health and emotional disorders. Program goals include:
- Reduced hospitalization
- Increased independence-of-living status
- Increased vocational/educational status
- Develop of natural support systems
Each consumer receives contact at a frequency that is based on their level of need. The need assessment may address any of the following areas:
- Living situation
- Vocational/Education
- Income/Benefits
- Health Care
- 24 hour crisis intervention
- Mental health treatment
- Socialization/Leisure
- Daily living skills
